Do I need a referral?

Yes, patients need to have a Primary Care Physician; However, referrals are also accepted by Medical Specialists. They will send a referral to our office via fax.

Do you accept my insurance?

Our providers participate with most commercial plans. We currently only accept Medicaid as a Secondary Insurance. HMSA Quest is not accepted.

Will I get prescriptions on my first visit?

No, your first visit is for a consultation only. Our providers do not automatically assume continued treatments for patients. The doctor will discuss treatment options available to you during your consultation.

Do you validate parking?

We do not provide parking validation. Waterfront Plaza’s parking rate is currently $3.00 per half hour or any portion there of. Should you purchase items from vendors downstairs, they can validate parking up to 1 1/2 hours.

What do I bring to my first appointment?

Please bring your completed Initial Consultation Packet, Picture ID (driver’s license or government issued ID) , and insurance cards.

What are your office hours?

Our office is opened Monday through Friday from 8:00AM to 4:30PM. We do not close during lunch hours.

Is wheelchair assistance available?

Yes, should you need assistance to our office from the parking garage or loading zone, please come 15 minutes before your appointment time and call our office to have someone come down and meet you with a wheelchair and provide assistance.
