The Honolulu Pain Management Clinic LLC accepts the following insurance plans:

We also accept cash payments.

 

HMO Plans are accepted but MUST have referral from a Primary Care Physician and have referral from the Specialist that is referring.
Workman Compensation referrals are accepted but MUST have a Treatment Plan signed by the Insurance Adjuster.
Medicaid Plans are accepted as a Secondary Insurance only; however, we do not participate with HMSA QUEST PLANS
Motor Vehicle Accidents Plans are NOT accepted.

Out-of-state work compensation carriers are Not accepted.
